I understand that participation in lacrosse involves physical activity that may result in injury. By registering for Swamplands events, I acknowledge the risks associated with sports, including but not limited to collisions, equipment-related injury, and environmental factors.
I authorize Swamplands staff, coaches, or medical personnel to act on my behalf in the event of an emergency, and to administer or seek appropriate medical treatment as needed for myself or my child.
I release and hold harmless Swamplands Lacrosse, its organizers, volunteers, sponsors, and venue partners from any and all liability for injuries or damages resulting from participation in any Swamplands-sanctioned activity or event.
I agree to uphold a standard of respectful conduct. This includes sportsmanship, appropriate language, and behavior that supports a safe and inclusive environment for all participants.
I grant permission for Swamplands to use photographs, videos, or likenesses of myself or my child for promotional purposes, including websites, social media, or print materials.
By checking the waiver box on the registration form, I confirm that I have read, understood, and voluntarily agree to the above terms on behalf of myself and/or my child.
